Mars Climate Orbiter

The Mars Climate Orbiter was lost enroute to Mars in 1999 due to a software problem. Data output from the telemetry system were in imperial (also known as English) units, but the guidance system assumed the units of the data were metric units.

This caused the Orbiter to be guided into Mars's atmosphere instead of into orbit around the planet. The probe subsequently burned up.
